I appreciate the trust you have shown in allowing me to provide for your medical needs.
The purpose of this letter is to inform you that I am leaving my practice at CCMH Medical Mall Clinic as I have accepted a position that is closer to my home. My last day will be April 28th, 2022.
CCMH Medical Mall Clinic has two physicians who are accepting new patients and are available to attend to your healthcare needs. These providers include Craig Wamsley, M.D. and Andrew Wyant, M.D. Both physicians have access to your medical records. Cedar County Memorial Hospital is also in process of recruiting a Nurse Practitioner or Physician Assistant as my replacement.
Please contact our office to schedule an appointment with any of these providers or to ask questions regarding the transfer of your care.
